The individual named below desires to participate in the activities sponsored by Tyler Goodson Youth Football Camp, Inc. in partnership with TG31 Mission in Motion Foundation, Inc. As lawful consideration for being permitted to participate in the Activities, and for the intangible value gained by participating, I agree to all terms and conditions set forth in this agreement.

I AM AWARE AND UNDERSTAND THAT THE ACTIVITIES ARE DANGEROUS AND INVOLVE THE RISK OF SERIOUS INJURY AND/OR DEATH AND/OR PROPERTY DAMAGE. I ACKNOWLEDGE THAT I AM VOLUNTARILY PARTICIPATING WITH KNOWLEDGE OF THE DANGER INVOLVED AND HEREBY AGREE TO ACCEPT AND ASSUME ANY AND ALL RISKS OF INJURY, DEATH, OR PROPERTY DAMAGE, WHETHER CAUSED BY NEGLIGENCE OR OTHERWISE.

I hereby expressly waive and release any and all claims against Tyler Goodson Youth Football Camp, Inc., and its officers, directors, employees, agents, affiliates, successors, and assigns on account of injury, death, or property damage arising out of participation in the Activities.

I shall defend, indemnify, and hold harmless Tyler Goodson Youth Football Camp, Inc. and all Releasees against any and all losses, damages, liabilities, claims, actions, judgments, costs, or expenses arising out of any third-party claim related to the Activities.

This Agreement shall be governed by the laws of the State of Georgia, and any claim shall be brought exclusively in the state or federal courts located in Suwanee, Georgia.

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

I, the undersigned parent or legal guardian, hereby grant Tyler Goodson Youth Football Camp, Inc. and TG31 Mission in Motion Foundation, Inc. the irrevocable right and permission to photograph, film, record, and otherwise capture the image, voice, likeness, and/or appearance of my child during their participation in activities organized or hosted by the Organization.

I grant the Organization the unrestricted right to use, reproduce, distribute, display, publish, and create derivative works from the Content, in any media or format now known or hereafter developed, including print and digital publications, social media, websites, promotional materials, advertising, press releases, and broadcast media.

I understand and agree that I will not receive any compensation for the use of the Content. I waive any right to inspect or approve the finished products. I release the Organization from any claims arising from the use of the Content.

This release is binding upon me, my heirs, assigns, and legal representatives. I acknowledge that I have read and understood the terms and agree voluntarily.
